Application Guidelines
- Please attach a completed application form together with a self-addressed envelope with sufficient postage. Applicant has to settle the underpaid items and collect mails at the Post Office in case of insufficient postage. Please be reminded that applicant has to collect the documents in person at YMCA for any absence of envelop.
- Information submitted will be cross-checked with system record. If there is any inconsistency, system record of YMCA shall prevail.
- Only record after the year of 2011 will be provided.
- Only one sport activity or competition is allowed in one application.
